Questionnaires can be included in your onboarding flows so that new clients automatically receive a form to fill out when they join. This lets you collect essential information like training history, goals, medical considerations, and dietary preferences without manually assigning anything.

Why is it useful?

When a new client signs up, you need key information before you can build their program. Manually sending a questionnaire to every new client takes time and is easy to forget. By including a questionnaire in your onboarding flow, it is assigned automatically, and the client can complete it right away while they are still engaged and excited to get started.

Step-by-Step Guide

Creating an Onboarding Questionnaire

  1. Go to Library > Forms > Questionnaires
  2. Click Create Questionnaire
  3. Name it something clear like "New Client Intake Form"
  4. Add questions that cover the essential information you need:
    • Training history (e.g., "How long have you been training?", "What does your current routine look like?")
    • Goals (e.g., "What are your top 3 fitness goals?")
    • Medical considerations (e.g., "Do you have any injuries or medical conditions?")
    • Dietary preferences (e.g., "Do you follow a specific diet?", "Any food allergies?")
    • Schedule availability (e.g., "How many days per week can you train?", "What times work best?")
  5. Click Save

Adding the Questionnaire to an Onboarding Flow

  1. Go to Automations > Onboarding Flows
  2. Open an existing flow or create a new one
  3. Add a step to Assign Questionnaire
  4. Select your onboarding questionnaire template
  5. Position it as one of the first steps in the flow
  6. Save the onboarding flow

How It Works for New Clients

  1. A new client joins through your sign-up link or you add them manually
  2. The onboarding flow triggers automatically
  3. The client receives a notification to complete the questionnaire
  4. They fill it out on their mobile app
  5. You receive the completed responses and can review them before building their program

Building a Reusable Template

Create one comprehensive onboarding questionnaire and reuse it across all new clients. Tips for a strong template:

  • Keep it focused on information you actually need to start coaching
  • Group related questions together (training, nutrition, lifestyle)
  • Use a mix of question types: Multiple Choice for quick answers, Text for detailed responses
  • Mark critical questions as Required and nice-to-have questions as optional
  • Test the questionnaire yourself to make sure it flows well and is not too long

Reviewing Onboarding Responses

  1. Go to the new client's profile
  2. Click the Questionnaires tab
  3. Open the completed onboarding questionnaire
  4. Review all responses to inform your program design

Things to Note

  • Questionnaires are one-time submissions, making them ideal for onboarding
  • For recurring feedback from existing clients, use Check-ins instead
  • You can include multiple questionnaires in the same onboarding flow if needed
  • Onboarding flows can also include other steps like assigning programs, habits, and welcome messages

Can I use different questionnaires for different client types?

Yes. Create separate onboarding flows with different questionnaires for different packages or client groups.

What if a client does not complete the questionnaire?

The questionnaire remains pending in their profile. You can send a reminder or follow up through messaging.

Can I edit the questionnaire after clients have already completed it?

Yes. Editing the template affects future assignments. Past submissions are not changed.

How long should an onboarding questionnaire be?

Aim for 10 to 20 questions. Enough to get the information you need without overwhelming the client on day one.
